Sports Physicals

FREE SPORTS PHYSICALS

Sports Physicals will be offered FREE of charge to all athletes of the Logan-Hocking School District on the following dates/times*:

High School Students: Wednesday, April 17, 3 to 5:30 PM
Middle School Students: Thursday, April 25, 4 to 6:30 PM

*If there are multiple students in the same household that attend both High School and Middle School, you may choose one date that suits your schedule best.

Please bring ALL COMPLETED Final Forms to the clinic. If forms are not completed, a PARENT/GUARDIAN must be present or your physical will NOT be completed.

Before your physical, you MUST:

• Set up account or update all information in Final Forms. Final Forms set up instructions are found on the Logan-Hocking Athletic website.
• Print out Physical form on Final Forms and bring it with you.
